Dear Parent/Guardian:

 

Attached are four forms required for your child to participate in sports at SLA.

Please be sure all information is complete, accurate and notarized if required.

All students MUST have these four forms completed, notarized and turned in before they are

able to participate in sports at the Student Leadership Academy.  In addition, each student must maintain a semester grade point average of 2.0 in order to participate in school sports. If you have any questions, please feel free to call at 941-485-5551.
